Officials Unlimited · Level 2 Screening

Request a copy of your background check

Use this form to request the Level 2 criminal history record check that Officials Unlimited conducted on you. We can release this record only to the person it belongs to.

Have this ready

  • Your legal name — exactly as you gave it when you were fingerprinted, not a nickname.
  • The email address you want the record sent to — check it carefully, this is where it goes.
  • Your date of birth — we use this to confirm we are sending the right record to the right person.

This record is yours alone. Florida law limits who may receive a criminal history record. We cannot send your record to a spouse, a coach, a league official, or an employer — only to you.

Your league does not have access to the record itself. They only see your eligibility status.

Your request

All fields are required.

What happens after you submit

  1. 1

    We confirm your identity

We match the name and date of birth you provided against our screening records.

  2. 2

We release the record to you

Your record is emailed to the address on this form within 1–2 business days.

  3. 3

    We log the release

Every release is recorded, as required for criminal history records. Nothing is sent anywhere else.

If your record is wrong

You have the right to challenge the accuracy and completeness of your criminal history record directly with the State of Florida, at no charge. This is called a Personal Review, under section 943.056, Florida Statutes. Call FDLE at (850) 410-7898 to begin. For the national portion, call the FBI at (304) 625-2000.

If you were found not eligible

You may be able to apply for an Exemption from Disqualification under section 435.07, Florida Statutes. Start by contacting your league president or compliance officer.
